Print this card out on regular paper or, if preferred, card stock paper.  Fold it in half along the horizontal line.  Print your name on the card.  Take the card to your Surgeon or Primary Care Physician and have them sign it.  For added protection, you can get the card laminated.  Prior to ordering, show this card to your server and they will act respectively in most cases and allow you to order from the lesser portion menus.  Enjoy your meal.  Always keep the card to yourself.
